New Patient Registration

Patients will be accepted onto the practice list if they live within the boundary area as agreed with the local health authority.  

To register you will need to fill in a New Patient Registration Form which can be downloaded (see the links below) and completed or obtained from the reception desk. You will also need to bring two forms of ID when bringing in your completed forms, such as your photo driving licence or passport along with a utility bill or bank statement showing your address details,

Register with the surgery online

We are using a new online service called “Register with a GP Service “that makes it easy to register with this GP surgery.

Just fill in this quick online form to start the process https://gp-registration.nhs.uk/C82671    

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Paper forms are still available if you need one.

Once we have registered you, we may ask you to come into the practice for a new patient health check.  Generally this would be if there is a delay in obtaining your records from your previous GP practice.

Although you will be allocated a GP, you can see any one of the GPs working at the Surgery.
